1. Creative Leaf may ask you to provide your name, address, telephone number, age, gender and other necessary personal information in the following circumstances:

(3) When you wish to participate in our satisfaction surveys, or monthly campaign
(4) When you apply for members only service
(5) When you participate in members only events
(6) When you fill in name list in store
(7) To make other inquiries or questions to us.
2. Personal information shall be strictly managed by appointing a person in charge of information management within the company and complying with the Personal Information Protection Law and other relevant laws and regulations.
3. Personal information shall not be disclosed to a third party without the permission of the individual, except in the following cases

(2) When you use a credit card company or financial institution to purchase a service or product
(3) When we give out information or announcements in the name of our stores
(4) When we give you information or announcements from our stores' tenants
(5) When we provide or disclose your personal information in accordance with laws and regulations.
4. We will use your personal information for the following purposes
(1) To provide information on internal newsletters and new services
(2) To provide after-sales service and products related information
(3) To provide campaign information.
5. If you request to correct, change, or delete your registered personal information, we will respond to your request as soon as we confirm that you are the one making the request.
6. These rules may be revised in accordance with the enactment or amendment of relevant laws and regulations.
In case of any discrepancy between the Japanese version and the English version, the Japanese version shall prevail.
